Comprehensive community nursing support including wound care, tracheostomy care, PEG and enteral feeding support, stoma care, catheter and urinary management, complex bowel care, diabetes support, skin integrity assessment, pressure injury prevention and treatment, and palliative care support.
Comprehensive continence assessments with clinical recommendations to support participant wellbeing and NDIS planning, personalised to each person’s goals and lifestyle.
Development of individualised care plans in collaboration with participants, families, GPs, and support staff to ensure care is safe, practical, and person-centred.
TLC Clinical Nursing provides a broad range of community-based clinical supports for participants with complex health needs.
Complex wound care and management, skin integrity assessment, and pressure injury prevention, treatment, and care.
Tracheostomy care, PEG and enteral feeding support, stoma care, catheter and urinary management, including flushes, and complex bowel care.
Diabetes management and support, palliative care and support, and other nursing interventions tailored to complex participant needs.
Hands-on High Intensity Skills training and development for support staff working with participants who have complex clinical needs.
Initial training is supported by periodic review and refresher sessions to maintain safe practice, confidence, and consistency of care.
Ongoing clinical oversight to support staff caring for participants living with epilepsy, diabetes, chronic wounds, and other complex client needs.
